Hormone replacement therapy (HRT) works wonders, but breast growth often eventually reaches a plateau. Many trans* women wish for more fullness to finally feel comfortable in their body and in their clothing.
At the Praxisklinik am Rosengarten, we know: a breast augmentation for a trans woman requires different planning than for cis women. The male chest is often broader, the chest muscle stronger, and the distance between the nipples greater. Our goal is not simply to insert implants, but to create a soft, natural "tear-drop" shape or a full round shape that harmoniously balances the broader chest and creates a beautiful décolleté.
Breast implants (The classic)
We use high-quality brand implants (e.g. Motiva or Polytech) with special gels that feel natural.

Position:
We usually place the implant under the chest muscle (submuscular). This is especially important in trans* women, since there is often little of one's own glandular tissue available to cover the edges of the implant. The muscle provides a soft transition and a natural feel.

Incision technique:We make the incisions inconspicuously in the new inframammary fold.

The hybrid breast augmentation (Composite Breast Augmentation)
This is often the gold standard for trans* women. We combine a silicone implant (for volume) with lipofilling (autologous fat).

Advantage:
With the autologous fat we can specifically pad the inner pole of the décolleté (cleavage) and "soften" the edges of the implant. This brings the breasts optically closer together, which is extremely advantageous with a broad chest.

Pure breast augmentation with autologous fat
If you do not want implants and wish for a moderate enlargement (approx. half to one cup size per session), lipofilling is the most natural option.
The prerequisite is that there are enough fat reserves available in other areas of the body (abdomen, legs).

We use exclusively high-quality, cohesive silicone gel implants from renowned manufacturers. These are particularly safe, shape-stable, and feel very natural. Depending on your anatomy and your desired result, we choose between round implants for more fullness in the décolleté or anatomical (teardrop-shaped) implants for a particularly natural profile.
After the surgery, you should wear a special compression bra for about six weeks. This supports the tissue and keeps the implants in the desired position. Most patients are presentable again after one to two weeks. However, you should refrain from sports and heavy lifting for at least six weeks.
Yes, breast augmentation with autologous fat (lipofilling) is an alternative or complement to implants. In this procedure, fat is removed from other parts of the body (e.g. abdomen or hips) and injected into the breast. This method is particularly suitable for patients who want subtle growth or wish to refine the result even further after implant surgery.
As with any surgical procedure, temporary numbness can occur. However, because we work in a tissue-sparing manner, sensitivity usually returns completely during the healing phase. In rare cases, the sensation may be permanently slightly altered, which we discuss in detail beforehand in the consultation.
Choosing the size is a collaborative process. For a breast augmentation, we take into account your shoulder width, your chest circumference and your height in order to create a harmonious overall appearance. In our consultation, you can test different sizes with trial implants to get a feel for your new body image.
In trans women, we place the implant in most cases under the large chest muscle (submuscular). Since there is often only little of your own mammary gland tissue at the beginning, the muscle provides an additional covering layer. This ensures that the edges of the implant are not visible after the breast augmentation and that your breast gets a soft, natural shape.
The most common technique is the incision in the new inframammary fold. As a result, the fine scar is later hidden in the natural fold under your breast and is invisible when wearing underwear or swimwear. Alternatively, in some cases the access can be made via the edge of the areola.
