Abdominoplasty

Tummy tuck for
gender-affirming
body contouring

Medically reviewed by: Dr. med. Timo Spanholtz

An abdominoplasty (colloquially known as a tummy tuck) is a surgical procedure in which excess skin and fat in the abdominal area is removed and the abdominal muscles are tightened. This procedure is one of the body contouring options that trans, non-binary, and gender-diverse people choose to improve body shape and reduce dysphoria in the torso area.

What is an abdominoplasty?

An abdominoplasty is a surgical procedure that:

removes excess skin and fat on the abdomen
tightens the abdominal wall (muscle and connective tissue)
shapes a smoother, firmer abdominal contour

The procedure can be particularly useful after pregnancy, significant weight loss, or in the case of muscle separation (rectus diastasis) – and can also relieve dysphoria in the abdominal area.

Why abdominoplasty in gender-affirming care?

For many trans and non-binary people, the abdominal area is a source of physical discomfort or dysphoria, especially when fat distribution or excess skin does not correspond to the desired gender characteristics. An abdominoplasty can help to:
✔ visually define the waist and torso
✔ remove excess skin that does not respond to diet or exercise
✔ tighten abdominal muscles that have changed, e.g., due to pregnancy, weight fluctuations, or hormonal treatments
✔ improve the overall waist-to-hip ratio, e.g., for holistically more masculine or more feminine contoured proportions

How does an abdominoplasty work?

Before the procedure

Before the operation, your medical history, physical examination, and goals will be discussed in order to determine the ideal plan.

During the operation

The surgeon makes an incision in the lower abdominal area, removes excess skin/fat, and can simultaneously tighten the abdominal muscles to create a firmer abdominal wall. The navel position is adjusted if necessary.

After the operation

  • You will usually wear a compression garment
  • Swelling and a feeling of tension are normal in the first few days
  • Pain is controlled with medication
  • Initial mobility (e.g., short walks) is encouraged early on

Am I a suitable candidate?

An abdominoplasty may be suitable for you if you:

  • Have excess skin or fat in the abdominal area that does not disappear despite a healthy diet and exercise
  • Want your abdominal muscles tightened
  • Are physically stable and have realistic expectations of the procedure
  • Want to achieve an aesthetically more defined torso through body contouring as part of your gender-affirming care

Abdominoplasty & Body Contouring

Abdominoplasty is often part of a larger body contouring plan, which may also include, for example, the following:

Liposuction on the abdomen, flanks, or hips
Combination with top surgery or breast augmentation
Fat grafting (e.g., Brazilian Butt Lift)
Tightening of other body regions

The scope is individually tailored to your goals and anatomical conditions.

Healing & Results

The first few weeks after the operation are crucial for the healing process:

0–2 weeks: Swelling, pain, and slight limitations in everyday life are normal

2–6 weeks: Activities can be gradually increased

3–6 months: The final contours become visible, swelling continues to subside

The scars usually fade significantly over time, especially if you use sun protection and apply scar care.

Risks & Safety

As with any surgical procedure, there are possible risks, including:
temporary swelling or bruising
infections or delayed wound healing
seromas (fluid accumulations)
uneven contouring or scar problems

A detailed consultation with an experienced surgical team reduces the risk and helps you make a well-informed decision.

Costs & Insurance

An abdominoplasty is often considered body contouring as part of gender-affirming care. Some insurance companies cover the costs if there is medical necessity and documented dysphoria – often including a medical assessment or documentation of the psychological burden. It is worth clarifying the insurance guidelines and requirements in advance.

Abdominoplasty and gender-affirming goals

Masculinizing details

  • A firmer abdomen defines a more masculine silhouette
  • Can help to emphasize a V-shaped torso, e.g., in combination with liposuction on the hips and flanks

Feminine contouring

  • Through targeted skin removal and muscle tightening, a narrower waist can be created, especially in combination with fat redistribution on the hips or buttocks

Conclusion – What you should know about abdominoplasty

An abdominoplasty is an effective surgical procedure for tightening the abdominal area and can be a valuable part of your gender-affirming care. With the right preparation, realistic goal-setting, and an experienced surgical team, you can achieve a more defined, contoured torso shape that better matches your body image.

Frequently asked questions about Tummy Tuck

We are happy to answer your questions about the treatment here.

Will the result of the tightening be permanent?

Yes, the removed skin cannot grow back. However, to keep the result looking this good in the long term, you should maintain a stable weight. Significant weight fluctuations after the surgery could stretch the remaining skin again. A healthy lifestyle is therefore the best support for your new body feeling.

Can the tummy tuck be combined with liposuction?

Yes, this is actually very often the case. This combination (lipoabdominoplasty) allows us not only to tighten the skin, but at the same time to shape the waist and the flanks. This way, we achieve a particularly aesthetic and holistic result for your midsection.

When is a tummy tuck a good idea?

A tummy tuck is ideal for you if you suffer from sagging skin or stubborn fat aprons on the abdomen that cannot be reduced through exercise or diet. This often occurs after significant weight loss. The goal is a flat, firm abdominal area and a harmonious body contour that better matches your desired appearance.

What happens to the belly button during a tummy tuck?

During a complete tummy tuck, the belly button is usually precisely cut around and, after the skin has been tightened, sewn back into its new, anatomically correct position. So it remains your natural belly button, but is aesthetically integrated into the new overall appearance.

Are the abdominal muscles also tightened during a tummy tuck?

If your straight abdominal muscles have separated (a so-called rectus diastasis), we can bring them back together in the middle and fixate them during the tummy tuck. This not only results in a flatter stomach, but also stabilizes your core and can define your silhouette in a significantly more masculine or feminine way.

How long is the recovery time after a tummy tuck?

You should allow yourself about two to three weeks of rest before returning to your everyday work routine. For a period of about six weeks, you will need to wear a special compression garment. This supports healing, reduces swelling, and helps your skin to adapt optimally to the new contour.

How does the scar run with an abdominoplasty?

The incision is made horizontally above the pubic skin, so that the later scar can usually be comfortably hidden in your underwear or swimwear. The length of the scar depends on how much excess skin needs to be removed. We always ensure fine suturing so that the scar can fade well over time.
