Hip Shaping (Hip Augmentation) for trans* women is a surgical procedure used to make the contour of the hips more feminine and softer. This procedure is often chosen by transfeminine, non-binary, or gender-diverse individuals who are striving for a more feminine-looking silhouette and want to emphasize their proportions in the lower torso area.
What does Hip Shaping for trans* women mean?
Hip shaping is a surgical-aesthetic procedure in which the volume in the hip and trochanter area is specifically increased to achieve a curvier, more harmonious figure. This can be done using different techniques – usually through fat grafting, less often through implants – and can be individually adapted according to your goals, entirely according to your wishes.
Goals of Hip Augmentation
The goal of a feminizing hip augmentation is to create a natural-looking, soft, and curvy silhouette that is typically associated with a feminine body feature. These include:
- Definition of the hip contour
- Gentle transition from waist to hips
- Emphasis on an "hourglass shape" (hourglass silhouette)
- Harmonious proportion between upper body, waist, and lower body
- Combination with other feminizing procedures possible
How does the surgery work?
In a detailed consultation, you discuss:
- your goals (e.g. subtle emphasis or pronounced curves)
- your physical prerequisites
- available fat deposits for transplantation
- possible combinations with other procedures (e.g. Waist Sculpting or Buttocks Lift)
This planning serves to define a realistic result that suits your anatomy and your aesthetic wishes.
The most common technique for hip augmentation is fat grafting (autologous fat transplantation):
- Liposuction: First, fat is gently removed from areas such as the abdomen, flanks, or thighs.
- Preparation: The fat is cleaned and prepared for transplantation.
- Injection: The prepared fat is specifically injected into the hip and trochanter area to build up volume and contour.
Alternatively, implants can also be used in selected cases, e.g. if there is too little of the body's own fat available or there is a specific shape requirement. The choice of technique is determined during the consultation.
The procedure usually takes 1.5–3 hours, depending on the scope, technique, amount of transplanted cells, and combinations with other procedures.
Healing & Recovery
After the surgery, you go through a recovery phase in which the following is typical:
- Swelling and hematomas in the treatment area
- slight pain or pressure sensation, which is well controllable with medication
- First mobility usually possible after 1–2 days
- Working (for sedentary activities) usually possible again after about 1–2 weeks
- Complete healing and final contour results only after several weeks to months
During healing, wearing compression garments is often recommended to control swelling and support the newly shaped contour.
Results
Results of a hip augmentation can include:
- gently defined hip curves
- harmonious transition from waist to hip
- fuller, softer hip proportions
- better balance between upper and lower body
Since transplanted fat partly "takes" and is partly reabsorbed by the body, the final result usually appears about 3–6 months after the surgery, when swelling has subsided and the tissue has settled.
Before the surgery
- Expect a consultation in which your anatomy, goals, and any risks are discussed.
- Non-smoking: Nicotine can negatively affect healing – stopping smoking before and after the surgery significantly improves the results.
- Live healthily: A stable physical condition promotes recovery.
After the surgery
- Compression garments: Help to control swelling in the first weeks.
- Rest: Avoid heavy physical exertion in the initial phase.
- Regular aftercare: Follow-up appointments with your team ensure success.
Combination with other feminizing procedures
Hip augmentation is often combined with:
- Hip and Body Shaping (for a defined transition from waist to hip)
- Brazilian Butt Lift (Buttocks Augmentation)
- Breast Augmentation / Top Surgery
- Body Contouring / Body Shaping overall
Such combinations enable a holistic feminine appearance – tailored to your specific goals.
Risks & Safety
As with any surgical procedure, there are also potential risks with hip augmentation, including:
- temporary swelling and bruising
- uneven contours or asymmetries
- infections or seromas
- Partial loss of transplanted fat cells
- rare complications due to anesthesia
An experienced surgeon specialized in gender-affirming body contouring reduces risks and helps to achieve a safe, satisfactory outcome.
Who is a suitable candidate?
Suitable are individuals who:
- strive for more feminine hip proportions
- have sufficient fat deposits for transplantation
- are physically stable and healthy
- have realistic expectations of the result and healing process
Even people with very slim body shapes can get a hip augmentation – in such cases, the planning can be adapted to the individual prerequisites, if necessary with combinations of different techniques.

Frequently asked questions about trans* Woman Hip Shaping
We are happy to answer your questions about the treatment here.
Theoretically, there are hip implants, but in our clinic we primarily rely on shaping with your own fat. Your own fat offers a significantly lower risk of complications, feels more natural, and leaves no visible edges or implant borders. In addition, by performing liposuction at the waist, we kill two birds with one stone.
No, the surgery does not alter the skeleton. Since the bony pelvis no longer grows after puberty, we focus on the soft tissue. By strategically building up fatty tissue over the pelvis and thighs, we visually simulate a wider, more feminine pelvis that perfectly matches your proportions.
Since these are living fat cells, they react like the rest of your body to changes in weight. If you lose a lot of weight, the fat cells on the hips will also become smaller. A stable weight is therefore the best prerequisite for maintaining the newly gained feminine shape in the long term.
"Hip Dips" are the natural indentations between the pelvic bone and the upper thigh. In trans women, these are often more pronounced, as the male-read pelvis is narrower. Through targeted hip contouring, we can fill these dimples with the patient's own fat, thereby creating a harmonious transition from the waist to the outer thigh.
The most effective and natural method for hip widening is autologous fat transfer (lipofilling). In this procedure, fat is liposuctioned from other areas (e.g., the abdomen or back), medically processed, and injected specifically into the hip region. This allows us to widen the contour and achieve a softer, more feminine line.
The scars are minimal. For the suction and injection of the fat, we only need tiny incisions of about 2 to 3 millimeters in length. These are strategically placed so that, after healing, they are barely more than small, light dots and are not noticeable in everyday life – even in a bikini.
This depends on your available donor fat and the elasticity of your skin. As a rule, a significant harmonization of the silhouette can be achieved in one session. If an extremely pronounced build-up is desired, a second procedure can be carried out after complete healing (after approx. 6 months) to maximize the result.
