Top Surgery (chest alignment / chest reconstruction) is a gender-affirming operation to remove breast tissue with the goal of a flat, masculine, or neutral upper body. One possible variant is Top Surgery without nipple transplantation ("No Nipple Grafts").

What does
"No Nipple Grafts" mean?

In a classic Top Surgery, the nipples including the areola are first removed and then repositioned and sutured back on as "nipple grafts." In the variant without nipple transplantation, the nipples are not reattached, so that the chest after the mastectomy has a smooth, uniform surface without projecting nipples. This option may be suitable for many patients who:

do not want nipples on the new chest, because this better suits their body feeling or identity, or because nipples are a source of dysphoria for them.

prefer a more minimalist, plain aesthetic result.

plan to use 3D nipple tattooing or other aesthetic elements later instead of surgically placed nipples.


Who is this option suitable for?

The decision against reattached nipples can be made deliberately based on personal identity, aesthetic preferences, or psychological reasons. Some patients report that they feel more physically aligned without nipples and that the result better suits their self-image. Important: The decision is individual and should be made together with your surgical team in a detailed consultation.

What does this mean for sensitivity & scarring?

Sensitivity: In the variant without grafts, the question of nipple sensation does not apply, as they are not reattached. Other areas of the chest, however, may still experience changes in sensation – this varies individually depending on the surgical technique.

Scars: The incision and scars run according to the chosen technique (e.g. Double Incision). The scar can vary depending on breast size and your anatomy.

  • Inverted-T or Buttonhole (sometimes with sensation preservation)

Do scars still remain with the no-nipple technique?

Yes, the horizontal scars typical of a double-incision mastectomy do develop. However, since no circular scars form around the nipples, the overall result often looks very clean and calm. With good care, the scars fade over time and can often be well concealed later through muscle training or tattoos.

Do dents form in the chest due to the absence of nipples?

No. During the operation, your surgeon takes care to model the tissue under the skin so that an even, flat contour is created. Since no "hole" remains, but rather the skin is closed smoothly, an aesthetic, masculine or neutral chest is created.

Is it possible to wear nipple prostheses later, after top surgery without nipples?

Yes, there are high-quality silicone prostheses that look deceptively real and are attached with a special skin adhesive. This gives you the flexibility to decide, depending on the situation, whether you want to have a flat chest or a look with nipples (e.g. under tight shirts). Alternatively, there is also the option of tattooing nipples.  We even offer this option at our clinic.

Can I get my nipples tattooed later?

Yes, absolutely! This is a very popular option. With medical 3D tattoos (areola pigmentation), strikingly realistic results can be achieved. The advantage: you can determine the position, size and color of your nipples yourself, down to the millimeter, after the mastectomy has completely healed. This is even possible at our clinic.

Does the health insurance coverage change if you forgo the nipple transplantation?

As a rule, forgoing the nipple transplantation has no impact on the health insurance coverage for your mastectomy. It is a surgical decision within the procedure. We will discuss your wishes during the consultation and document them in the surgery plan.

Why do some people choose a mastectomy without nipples?

The reasons for a "no-nipple" mastectomy are varied. Many appreciate the aesthetic freedom – for example, for large-scale chest tattoos that are not interrupted by nipples. Others prefer a completely smooth, minimalist result or want to avoid the risks of a nipple transplant (such as healing complications or pigment loss). For some non-binary people, this look also better matches their sense of their body.

How does healing work without nipple grafts?

Healing is often less complicated and faster with this method. Since no free nipple grafts have to heal in place, a critical part of the aftercare is eliminated. You don't have to worry about the "survival" of the nipples or about crusting at that spot. Care focuses purely on the horizontal main scars.
